This consent is to document that the patient (or guardian) has voluntarily agreed to receive drug administration at home by a qualified nurse/healthcare professional deputed by the home care service provider.
Declaration and Consent
- (Patient/Relative/Guardian), hereby give my full consent for administration of the above-mentioned drug(s) at my home.
- I confirm that the drugs have been prescribed by a registered medical practitioner, and I have shared a valid prescription with the home care nurse.
- I understand that the home care nurse will administer the drug as per the prescription and protocol, and the home care company or its staff shall not be held liable for:
- Any adverse reaction, side effect, or allergy caused by the drug.
- Any deterioration in the patient’s condition arising from the underlying disease or response to the treatment.
- Any emergency situation occurring during or after administration.
- I confirm that I have been informed about the possible side effects, and I take full responsibility for contacting my doctor or emergency services if any adverse reaction occurs.
- I understand that the home care service is limited to drug administration only and does not replace hospital-level monitoring or emergency medical support.
- I release the Home Care Company / Nursing Agency and its employees, nurses, or associates from any claim, demand, or liability arising from or related to the drug administration procedure conducted at home.

6. Bookings, Cancellations & Payments
When you submit a service request, callback form or booking through our website, you authorise us to contact you through phone, SMS, messaging apps or email to understand your requirement, confirm details and share next steps. All bookings are subject to provider availability, location feasibility and medical appropriateness.

At present, most online payments for our services are processed through Razorpay (India) using its secure payment gateway and webhook-based confirmation. When you make a payment, certain information such as your name, contact details and transaction-related identifiers are shared with Razorpay so that it can process the payment in accordance with its own terms and privacy policy. We do not store your full card number, CVV or netbanking passwords; these are handled directly by Razorpay and/or its acquiring bank partners.
An order or booking is treated as confirmed only after we receive a successful payment status from Razorpay via its server-to-server webhook or equivalent backend confirmation, even if your browser shows a preliminary “success” message. In the event of any mismatch between what is displayed to you and the status received from Razorpay, our records based on the Razorpay logs and confirmations will generally be treated as final, subject to applicable law and any genuine dispute-resolution process.

Final charges may vary depending on the actual time spent, consumables used, additional procedures requested, or complexity of care required, and you agree to pay such reasonable and transparently communicated variations.

- Provide complete, accurate and up-to-date information about the patient, including medical history, medications, allergies and current symptoms, to the best of your knowledge.
- Ensure that your home environment is reasonably safe and hygienic for visiting doctors, nurses, attendants and technicians.
- Treat caregivers, volunteers and support staff with respect, dignity and basic courtesy, in alignment with the values of “Hari Om Mother Father”.
- Follow the instructions and care plans recommended by your doctors and healthcare providers, and promptly inform them if there is any change in the patient’s condition.
- Use any equipment, devices or consumables provided as part of the service exactly as directed and not for any unauthorised purpose.
- Ensure that a responsible adult family member or authorised attendant is present at home during visits (unless otherwise agreed in writing), to receive the provider, share accurate information, and support decision-making for the patient.
- Inform us in advance, as far as possible, if there are contagious or infectious conditions in the household, and follow reasonable infection-control precautions suggested by our team (for example, mask usage, ventilation, limiting the number of people in the room).
- Refrain from audio or video recording, photographing or live-streaming the visit without the clear knowledge and consent of the healthcare professional, except where mutually agreed for medical record-keeping and in compliance with applicable law.
We reserve the right to decline or discontinue services if there is abuse, harassment, unsafe conditions or repeated non-cooperation that puts our team or partners at risk.
8. Caregivers, Partners & Third-Party Providers
To serve you better, we may collaborate with independent third-party providers, including doctors, nurses, paramedics, laboratory partners, radiology providers, ambulance operators, volunteers and technology vendors.
